• Death of a CJA attorney/service provider - Vouchers should be submitted by a personal representative or executor of the attorney's or service provider's estate. Payment should be made payable to the attorney's "estate." The personal representative should also submit a Standard Form 1055, "Claim Against the United States for Amounts Due in the Case of a Deceased Creditor."
