Abstract: 

Root Cause Analysis (RCA) is a system of investigation designed to identify fundamental factors, or root causes, responsible for errors in performance within a system or practice. The authors examine the potential costs and benefits of the RCA approach in the human services sector from a correctional perspective, based on experience gained in implementing RCA in the Queensland (Australia) correctional oversight system.
